Conwy Disability Network is a joint project between Education, Health, Social Services, Voluntary Organisations and the Parents/Carers of children and young people (0-19) who have a disability or chronic illness. Its aim is to enable the effective planning and coordination of services.

Who has Access?
The Network is held on computer by Conwy County Borough Council. Only the network coordinators and those professionals from health, education and social services who work with the child or young person have access to individual information. All individual information is confidential and is not given to anyone else. General information, such as the number of children or young people in the network (but not their names, addresses and personal details), is seen by the professionals outlined above and may be included in the local authorities Children’s Services Plan to help improve services.
How do I change or remove details from the Network?
You can ask to see the information about your child at any time and change any details that are incorrect. You can also ask for your child’s details to be removed from the network at any time.
